Letter to Patients
Dear Patients and Families,
After 34 wonderful years of caring for the smiles of children in our community, I am writing to share that I will be retiring and closing the office for Dentistry For Children and Teens effective June 30, 2026.
This decision comes with mixed emotions, as it has been an incredible privilege to watch so many of your children grow over the years. From first visits and loose teeth to milestone checkups, being a part of your family’s dental journey has meant more to me than words can express. It has been an honor to care for your children and to build relationships with so many wonderful families. Your loyalty, kindness, and support throughout the years have made this practice truly special.
As I step into this next chapter, I want to ensure a smooth transition for your family’s continued care. Your dental records will remain securely maintained, and a patient record transfer form can be requested via text to us at (330) 385- 6201. Please note that most offices will only request records from the past 6–12 months; if your child has not been seen within that timeframe, your new dental office just create new records with their office.
Your child’s ongoing dental health remains very important to me, and I encourage you to continue regular checkups with your new provider. Dr. Wellington is willing to accept as many Dentistry for Children and Teens patients as he can. If you would like to continue dental care with him please contact his office at (330) 385- 1352 to confirm his office accepts your insurance plan. You may also refer to the back of your insurance card for a list of in-network providers.
Thank you for your kindness, your trust, and for allowing me to be part of your children’s lives. I will truly miss seeing your familiar faces, but I leave with a heart full of gratitude.

You can request a record transfer by texting us at (330) 385-6201 and we will send you a link you can fill out a form directly from your phone and submit it to us, or by emailing us at dent4childteensoh@msn.com and requesting the form via email. If you request the form via email you will need to print the form and scan it back to us.
The best way to contact the office with any questions is to text (330) 385-6201. We will have staff monitoring the messages. Please allow 24-48 hours as staff may not be in daily once the office is closed.
Every office is different. If you have been seen in within the last 6 months they will likely want any x-rays that may have been taken. If you haven’t been seen for a year or more they may just start you as a new patient entirely and not want anything from us. Talk to the office you will be transferring to and ask them what they want from us and we will be happy to send the information over to them after a record release is signed.
If your child was scheduled for a surgery with us those appointments will not be affected by the office closure. Those appointments are done at the hospital and will continue as scheduled. If you have any questions please text (330) 385-6201 and a staff member from the dental surgery department will get back to you.
You can call your insurance company and ask them when your child’s last cleaning was. Your child will be due 6 months from the date of their last cleaning.
You should have been notified of your appointment being cancelled already. However, if we were unable to get in touch with you we apologize for any inconvenience, but appointments after June 30th are cancelled. However, you can use the date your child was scheduled as the date they will be due with a new dentist.
